In partnership with University of Texas Brownsville TeleHealth Center, this program is designed to address the epidemic of diabetes in the Lower Rio Grande Valley. The incidence of diabetes in the Valley is three times the National average, and nearly one in two Hispanic children will develop the disease in their lifetime The solution is to partner with the University of Texas Brownsville to develop a diabetes healthcare center and to leverage the healthcare center with a patient outreach program called Virtual Health Check VHC provides a new standard of care for patients with chronic disease. It combines wireless blood pressure, weight and glucose information with regular patient outreach. The patient outreach calls review the physician's protocol and help the patient manage their disease. The vital sign and patient contact information is uploaded to a Web application for physician review. Patients with abnormal vital signs are seen in the clinic. The goal is to reduce acute care episodes and help patients better manage their disease.
